Ordering MicroGenDX Test Kits

From the MicroGenDX website for your office:

How to have kits shipped to your patients:

1 – Call Customer Service and give them all needed patient information. They will contact patient and charge $15 shipping to patient. Patient pays test price when submitting sample.

2 – Have your patient order their own kit online using your practitioner MicroGenDX code number on their requisition form (this is set up by customer service and emailed to you monthly). If the monthly code has expired or the patient doesn’t enter this, you won’t get their results attached to your account or have it appear in your results portal.

3- Order the patient’s test kit(s) through Femologist.com. Contact Ronen@femologist.com for initial set up. You will need to email the patient the needed requisition form with their specimen. (See forms above.) Now patients can receive the needed biofilm disrupters and/or products along with their test kits and multiple kits can be sent without a code that might expire before the kit is needed.

Insurance Information

Please see updated list of insurance carriers for MicroGenDX testing. As always, it is best for the patient to double check with their insurance company since this information often changes.
The MicroGenDX fill-able requisition forms (avaiable on this page above) can be emailed to the patient to include with their sample instead of them using the MicroGenDX codes that expire monthly. This will ensure that both you and I get the report.
There is also an attached list of Z and R codes that can now be used. This may increase insurance coverage for some patients.
