Step 1 – visit MicroGenDX site
Visit https://microgendx.com/ and select Provider Portal option on top right corner. Or directly go to the Provider Sign Up page at the following link or uset the button below – https://providerportal.microgendx.com/signup

Step 2 – At Account Sign Up: Enter your Information and [CRITICALLY] enter Ruth Kriz, APRN as your Salesperson
In order to be added to Ruth Kriz APRN account, you must enter the correct Name of Salesperson on the account sign up form.
Step 3 – Complete Registration and Access your account or order kits
You may be asked for additional information such as your NPI or other identification at this point. Continue entering your information and submit your form.
Once registered, you can go back to the MicroGenDX.com site and log in (if you are not already logged in).
Now you are ready to order kit supplies directly to your office or to your patient. Click the button below to learn more about the options to order kits.
Step 4 – Access Requistion Forms for your Patients, Insurance Information and Patient Instructions you can share.
Click on the button below to get access to the requsition forms for your patients. Instructions by Ruth Kriz, APRN to share with your patients are also avaialble. Up to date Insurance coverage information is also available.
